Funeral Benefits
If an Insured or a Family Member dies, benefits elected in writing to Chaba Di Kopane By the Insured in terms of the policy, will be given. The basic funeral plan provides for a funeral benefit to be executed out in settlement of a death claim of a Principal Member and his/her nominated dependants. Please note: Your policy will only be active from the date of the first premium payment received by the underwriter (which will be the first 1st of the following month). Irrespective of the entry date on the original application form or the entry date on the first page of this document. Benefits will be given according to age(refer to the benefit page)
Client
Any person in respect of whom Chaba Di Kopane arranged funeral aid insurance in terms of the policy.
Insured
Any client under age 65 at the time of qualifying for the insurance who is insured in terms of the policy
Still-born child
A child that has had at least 26 weeks of intra-uterine existence but showed no sign of life after complete birth. Still born will only be covered with burial only, no other benefits.
Extended Family Member
Any person, except a Qualifying Child or a Qualifying Spouse, who is dependent on the Insured for maintenance, is under age 85 at the time of qualifying for the insurance and is nominated in writing to Chaba Di Kopane by the Insured and for whom an additional monthly premium as determined by Chaba Di Kopane is paid. The insured must provide proof that the person(s) are in fact dependent on him/her. A qualifying parent and additional spouse need not be dependent on the Insured for maintenance.
Commencement of Insurance
Insurance commences on the first day of the month in which the insurer receives the first premium, which will be the 1st of each month.
Premiums
A monthly premium, determined by Chaba Di Kopane, is payable monthly . If any premium is not paid timeously, Chaba Di Kopane liability in terms of the policy regarding that Insured and his/her Family Members lapses.

The Chaba Di Kopane policy should be paid by the 10th of each month, failure to do so,30 days grace period will be given

Failure to pay premiums within sixty(60) days your policy will lapse,new six(6) months waiting period will apply

The premium amount should be paid each month. This policy covers you on a monthly basis, if the premium is not paid it means you’re not covered, and you’re still liable for outstanding premium to resume your policy.

If your policy has not being paid for three consecutive months it will lapse, therefore new six months waiting period will start.

If your policy is not paid for three months, your policy will lapsed, and if you decide to rejoin a new waiting period will start.

It is the clients responsibility to ensure that they have used a correct reference number, when making a EFT payment , wrong or unidentified reference will be treated as non payment, until proof of payment has been submitted

Chaba Di Kopane reserves the rights to adjust the premiums if necessary, which will not be more than 20% per annum.
Exclusions
No claim will be admitted in terms of this scheme if the event giving rise to the claim is caused directly or indirectly by or is in any way attributable to any of the following:
The willing participation by the member in any of the following:

illegal military action
-
insurrection
-
terrorism
Any act or deed by the member deliberately committed in violation of criminal law. All this will be treated as suicide claim.
Waiting Period
In the case of death due to natural causes, cover or any increase in cover has a waiting period of 6 months. If the Insured’s/Family Member’s benefits have lapsed and he/she again becomes insured in terms of the policy, the above waiting period will apply again. In the event of a discrepancy between the policy summary and the policy itself, the provisions of the policy will prevail. The policy may contain terms and conditions that are not dealt with in the summary. Suicide claims will only be honoured after 24 months of joining.
Claims Procedures
In the event of a death, a Claim Notification Form must be requested from Dignities, and must be submitted together with relevant supporting documents within 6 (six) months of the date of death. Failure to do so within the 6(six) month period from date of death, will result in the benefit being forfeited.
Should Chaba Di Kopane pick up the corpse, and you decide to use another undertaker, you will be liable for the charges, charged by Chaba Di Kopane This is funeral cover and not a cash cover, if Chaba Di Kopane is not doing the service for you, you will not be liable to claim 100% of your cover, but only the amount due to you as stated on benefits page(refer to benefits page
Family cover
All our family covers take member plus spouse under the age of 65 and children under the age of 21 to maximum of 10 lives covered on one policy . Should there be more lives needed to cover they will fee charged . If any of the children becomes over 21 years of age , a fee will added and it is also member responsibility to inform Chaba Di Kopane if a child becomes 21 years as this will result to no cover if there is no premium attached . This cover can add people over 21years not older then 64years as extended at additional cost
Member+9 / unlimited
This policy covers 9 nominees and main member . Main member should be under the under the age of 65 nominees should be under 74 years . For our bronze plus {R210} and silver plus {R370} cover nominees should not be older then 84years . Should you wish cover more then 10 people to maximum of 14 people , additional cost will be added ,additional cost will be added , no premium no cover for all extra people
Grocery cover
You only get grocery in case of death claim .Grocery will be given according to age , this cannot be exchanged with cash . This is not burial cover you only get grocery in case of death claim
Cash cover
This covers you with cash only in case of death . 0 to 5 yrs = 5% of cover , 6% to 13 yrs = 25% of total cover, 14yrs to 74yrs =100% of cover
Tombstone cover
If u choose not to be buried buy CDK you can receive tombstone money based on covers they choose
Promo Product / Floor Crossing
All promotion consist of no waiting period for burial service only , meaning you will receive coffin , mortuary, tents and registration of death . All other benefits such as grocery and kgomo {beef} you will only qualify after six months
Transportation
Groceries will be delivered to the nearest office. It will be charged for home delivery add an extra fee.

Certified copy of deceased’s ID document.

Confirmation of bank account from the bank, not older than three months

Copy of Membership Certificate.

Certified copy of marriage/registration certificate or sworn affidavit that the principal member is mar-ried to the spouse or that the principal member has lived with his/her partner for at least 6 months.

Full time student: Letter from recognized educational institution.

Disabled child: Confirmation of disability grant and a copy of medical report relating to disability.

In the event that a child’s surname is different from the principal member, an affidavit is required to explain the nature of the relationship to the principal member.

Stillborn child: certified copy of death certificate and the Notification/Register of Death/Still Birth (83/B1 - 1663) form, as well as a letter from the doctor/hospital in attendance, confirming the duration of the gestation period.
In the event of an unnatural death, a copy of the Police Statement must be submitted with the Claim.
Claims, in respect of Family Members, will only be paid where such Family Members have been nominated on the original application/amended form. The underwriter reserves the right to request any further documenta-tion or information as it may deem necessary to accurately assess a claim.
The underwriter will endeavour to settle the claim within 48 hours of receiving all of the required fully completed documentation. Faxed copies must be clearly certified. The details of the Commissioner of Oaths with all the relevant details must be clearly legible. Documentation submitted, other than those listed, will not be accepted, EXCEPT WHERE SPECIFIED ABOVE.

There are no surname restrictions.

When conduct a funeral, we give 50km radius and above that the family should contribute from their own pockets

All application forms must be filled in completely including ID numbers, and entry dates.

Should you wish to replace/ add a member a new waiting time will start.

Family members on application are only valid with main members’ signature/agreeing to the declaration
-
Waiting period will start from the day ID copies are submitted, or the day you agree/sign the declaration that you confirm that ID numbers are correct

In a case whereby copy of identity document is not provided, client will take full responsibility of member not being covered due to the wrong id number. This will result in member’s claim not be honoured

No money back if the member decides to leave the Chaba Di Kopane

The benefit will differ according to age (refer to the benefits page).

It is the main member’s responsibility to ensure that dependants are not over insured/ registered, as only fifty thousand rand can be paid for one person.

Still born and 12 months babies will only be covered with burial only, only for members completed their waiting period, provided you have space on your policy,if number of lives covered are not full according to the cover you chose.

Suicide claims will only be honoured after 24 months of joining.

If there are any changes you want to make, (cell phone numbers, address, member replacement, etc), please make sure that you inform us The maximum entry age for member and /or nominated family dependents is under 65/70/75 years (depending on the selected option). Cover for all nominated family members cease at 99 years. (Children and extended family members included).

Unmarried mentally or physically disabled children who are totally and completely dependant on the main member will be covered for as long as the policy is in force.

Cover will be provided for a maximum of 9 or 14 nominated family members. (E.g. Spouse, additional Spouses, Children, Dependent Children, Extended Family Members).

Once the principal member’s cover ceases, the policy can be taken over by any nominated family member.

For all cash premiums keep receipts as proof of payment.

The Underwriter reserves the right to cancel the underwriting with 30 days’ notice.
Death declaration
As its required by law for a medical practitioner to certify death,there will be an extra fee that must be paid by family, if the deceased died from home or before being checked by a medical practitioner
